Important Forms

The following forms are for your record and for the School of Hospitatlity. You are responsible for meeting the course required 600 hours as well as tracking your own hours. These two forms: Form "A" or Summary Chart of Hours and Form "B" or the Individual Organization Hours Chart are used as a tool to track, measure, and evaluate your experince. However, only HM approved jobs count toward your total work expereince. Please keep these forms and your hours neatly aranged and stick to these formats. You can download and print the following documents, fill in required text, and turn in on specified due date.

Please Print:

(1) Form "A" or the Summary Chart of Hours and

(1) Form "B" or the Individual Organization Hours Chart for each company or organization listed on Form "A"

Due Dates

For students enrolled in the Fall, forms will be electronically submitted through Canvas by 5pm October 15th.

For students enrolled in the Spring, forms will be electronically submitted through Canvas by 5pm February 16 th.

The examples below give you an idea of the forms, Summary Chart of Hours and the Individual Organization Hours Chart, and help you get a better idea of how they are to be completed. They are also used to give you an example and overview of the accepted positions HM positions. The Individual Organization Hours Chart also has a short answer section which asks about you positions duties and responsibilites. These questions are displayed to help you reflect on your experience.
